It reallllly is pretty heart-breaking, and I somehow think it may be even more so with it "extended" (somehow or other) on-screen. I think he knew he had to go as long as he could, but... I don't know if he thought it would definitely kill him at the time, or if he realized it for sure later, when it took them so long to get to help :-( Either way, it's really sad.
